Until the cervix actually dilates, nothing can be told about incompetent cervix by examination. The length of the cervix is from the internal os to the external os. Examination does not allow you to feel the internal os until after the cervix is dilated and that is too late. No one should worry about incompetent cervix unless they have had a previous late 2nd trimester loss where first the bag of water ruptured and they were found to be several centimeters dilated and then started having contractions after that.
