12 weeks or 14 weeks?

Usually it is heard that if you pass the 12 week mark, you can breathe a little sigh of relief. However, recently, I have heard of people who have had m/c's at 14 weeks.

Do you consider 12 or 14 weeks to be out of the first trimester since you are really pregnant for 10 months?

Would the 14 week be a chromosomal m/c or a problem with the mother?
