prenatal diagnosis

I actually have three concerns.

One, at thirty weeks, what kind of testing can be done to determine the likelihood of a baby with genetic birth defects such as down's syndrome, spina bifida, etc. I do not have a history in the family, I am 32, husband is 35, and am hoping I can still do low risk testing, although I realize at this point in the pregnancy options become more limited, if there are any options.

The reason I ask is my second concern. Partly because I have not been agressive enough in keeping track of what my doctor is doing, I am just finding out that the maternal serum testing results that should have been done way back when were lost somewhere. Is there a list of questions or some kind of checklist that a patient can refer to so they know what should be going on and when with pre-natal care? I am upset, needless to say, for I feel up to this point the information I have been getting from my doctor I had to ask for and he isn't one for "sharing."

Finally, does anyone have any recommendations for improving this doctor patient relationship? I may want to change to a new doctor or midwife, perhaps one within the same practice, and worry about the effect of this as well.

Thank you for your time.
