Jenny - Before we toured the birthing center of our hospital, My husband and I went to, I believe, babycenter.com and parentsplace.com and looked at the various fill-in-the-blank birth plans. Then we took a hybrid of the two with us and asked the nurses about all the things we didn't understand on it - including, is it okay if we have a birth plan? Even though there are some things only your doctor can answer, the nurses seemed to appreciate being asked and being allowed to explain things from their point of view. One note - the nurses seemed VERY nervous about the idea of a birth plan until we explained that it wasn't written in stone, it was mostly to help us understand the process and that we would bow to the experts. Dr. Shanahan's book, "The Over Thirty-Five Week by Week Pregnancy Guide" also talks about basic birth plans and what to ask. She makes a good point that some things on birth plans are silly (though how would we know if we didn't ask someone?) and to be careful to state your wishes without saying "never" or "I will not".

Our current version of the birth plan has more questions than statements - things the nurses didn't know or questions about explanations I didn't understand, and I'll be asking those questions of my OB when I see her next.

I hope this helps.
