large for dates - what questions should I ask at next week's ultrasound?

At my past two check ups, the midwife (I see a different one at each visit) has mentioned that my uterus is quite large for where I am in my pregnancy and that I must be further along than thought. I don't think this is possible since I had a sono at 6 and 10 weeks - both times the sono showed just one baby and coincided exactly with my pregnancy's dating. I am currently 18 1/2 weeks and my uterus is about two finger widths above my navel. I have several questions. First, how abnormal is this? Second, what problems could be causing such rapid growth? Finally, at my 20 week ultrasound (scheduled for 10 days from now), what questions should I be asking to explain this growth? Although it may not be related, I am experiencing very little fetal movement. This is my first pregnancy. Thanks for any info you can provide.
