fibroid --action or inaction

I just turned 43 and have a very large fibroid on a stalk outside my uterus. I had this huge fibroid (it was mistaken for my baby's head) during my last pregnancy. That pregnancy required a c-section but went well.

I now would like to get pregnant again. My doctor would like me to have the fibroid removed first. Is it necessary? My insurance will not pay for the operation and I don't know if the risks of the operation are that much less than the risk of pregnancy with the fibroid? Also, the idea of having major surgery when the chances of conceiving are so low to begin with concerns me. Help please.
