I have been trying to conceive for the last 18 months and so far without any success. My periods are regular (29-30 days, duration 5 -6 days). Started at the age of 14 and never missed one. After the completion of one year I, and my husband have undergone various tests and advices. 1)Semen Analysis - The result was Qty - 4ml, color- gray white, consistency -viscous, reaction - ph -8, fructose - +, sperm count - 125 million, motility - 82%, red blood cells - absent, few epithelial cells seen, occasional pus cells seen, no abnormal spermatids seen. 2)PCT on 12th and 14th day of my menstrual cycle. But no motile sperm was seen. 3) Laparoscopy, Hysteroscopy and EB. Clinical Findings and Summary: Ovaries Rt. and Lt. (w) developing follicle (R )(+) , POD - no c/o Koch / adhesion, single brown spot (+) on the Lt. Uterosacral ligament. MBI - bilateral free spills (+). HPE - Multiple tissu bits show straight tubular endometria glands lined by tall columnar psudostratifed epithelium. The stroma is compact. Cytoplsmic vacuoles/intraglandular secretions/stromal decidualisation/ spiral arterioles are not seen. No evidence of kochs. No evidence of atypia / hyperplasia / malignancy. Features are suggestive of prolipherative phase endometrium. 4) Follicular studies - a. The uterus measures 8.1 x 4.6 x 3.6 b. Right ovary measures 3.3 x 1.5 cm. Left ovary measures 2.4 x 1.2 cm. Day 11 to day 17 (Dominant Follicle on 17th day) 5) Blood tests a. Prolactin - 98 ng /ml. When retested three blood samples at an interval of 30 minutes 23.39 ng/ml. b. Thyroid Stimulating Hormone (TSH) 1.29 ulU/ml. c. Progesterone 24.70 ng/ml. After that I underwent PCT again on 16th day 10hrs after intercourse. Motile sperms were found in the mucus. Please advise what is next.
