in my town, a short scribbled note usually suffices but here is a more persuasive note:

Dear Jury Commissioner Ms. _________ is currently 10 weeks pregnant, after a long and difficult course. She requires absolute freedom of movement and unrestricted access to a bathroom at all times. She needs to eat and drink every 2 hours. If these conditions are not followed, I cannot guarantee she will not 1) vomit or 2) urinate all over your jury box.

I respectfully request you excuse her from jury duty for the duration of her pregnancy.
