I had an episiotomy that was still causing pain 7 months post-partum. There was only minimal excess granulation there. My doctor prescribed oestrogen cream and I also did some deep tissue massage. Both of these combined seem to have helped with the pain. The deep tissue massage especially relieved the burning I had experienced with intercourse, the oestorgen I think helped with improving the natural lubrication of the area and softened the scar somewhat. It has improved enough that I'm no longer considering surgery to get it re-done (besides I'm pregnant again so I'll se what happens this time around). If you need the surgery, have it done, it is better to have short term discomfort than to always be uncomfortable and never enjoy sex again.
