Last menses vs. conception

When using a baby dater provided by the doctors office it shows last menses and conception date. If you know your last menses, but also have a possible date for conception how does that adjust how many weeks you are? Does it automatically reduce it by 2 weeks? I am confused at to how to count, and the more I look the more confused I get.
