Every MD is different. I use clomid for 6 months only, increasing the dose every 2 months. The level of 11.5 is ok, but I usually try to get it up higher, upper teens to 20's. Changing the dose can be beneficial and the rule of thumb is that you need doses that mirror your weight, with thin women usually responding to 50 mg, heavier women needing 100mg, and so on. Doses of 200 and 250 are used, but by then, I have usually sent the patient to a fertility specialist. Hope this helps.
