13.5 wks preg - OB says uterus too big. What is it?

I'm pregnant with my third child now. I'm 13 1/2 weeks pregnant - and I'm 100% sure about my dates. (LMP 7/2)Periods regular, I can feel when I ovulate, etc. Anyway, at my last visit,we finally heard the heartbeat (other pregnancies heard hearbeat at 9-10 wks) and the OB said my uterus on external palpation felt like it was the size of a 16-17 week gestation. I thought it might be because I am skinny and there's no fat for the uterus to hide under. But OB disagreed. OB was concerned and ordered a sono for next week. What could be the reasons for this large uterine size? I doubt it is twins. No family history, no fertility drugs, etc. My breast have been and continue to be unusually tender (more so than previous pregnancies)and I am throwing up ocassionally (never did that in other pregnancies). What could large uterine size at this stage mean? fibroids? molar preg? I'm a little worried because the OB seemed concerned - but I have to wait a whole week before my sono appointment.
