C/S or VBAC (for SIL)

My sister-in-law had her first child by C/s in March 1999. Here labor was induced, horrible pitocin contractions but she made no progress and was getting no relief from the epidural. The had to use GA for the surgery and she delivered a 8#11oz girl at 39 weeks. She ended up with a nasty wound infection that took 3 months to completely heal. I presume the wound complication is probably related to the fact that she is very big (probably pushing 300#).

She is pregnant again and due in December. Her first OB just wanted to schedule the c/s and be done with it. Now she has a new OB who is willing to entertain the possibility of a VBAC. SIL is not really sure if she even wants to try it, and the doctor has indicated that they have to wait until later in the pregnancy to decide for sure if it's even an option. But the new doc has expressed a concern that the previous incision is "tender".

Does the fact that she had a post-op infection put her at higher risk for rupture? Does her size present an additional complication for VBAC? Or the 2 pregnancies being fairly close together?

If she wants to try a VBAC, are there any key factors she should check on in choosing the hospital or the doctor? I guess I'm looking for a list of conditions that should be met by the doctor or hospital if she is going to try a VBAC.

And of course, the whole thing is emotionally complicated because her mom is basically anti-doctor and bawled over the last c/s and really thinks the doctors just want to do a c/s to make it easier for the doc and collect a higher fee, blah, blah, blah. She just hates the idea that the baby may not be born vaginally "the way the Lord intended". She has never been pregnant (both her kids are adopted) and is not very knowledgeable about the whole process. I have tried to educate her about the possibility (however remote) of a rupture and just how catastrophic that can be and that SIL and the father really have to make this decision with their doctor. She just nods her head at me like I'm the fool being led by heathens. I think she just lacks the understanding to acknowledge the possibility that a c/s may be safer for everyone. And she blames the doctor for the 1st c/s -- that everything would have been fine if they hadn't induced and just let nature take it's course. I'm sure ya'll know the type.

Any feedback to help SIL make a more educated decision would be appreciated.

Thanks, Andrea
