Insulin w/GDM

I take medium acting Insulin to control Gestational Diabetes because my after lunch blood sugars have been consistantly elevated. My problem is that the insulin has lowered my after breakfast and after dinner blood sugars (which were fine to begin with), but since I have been on the Insulin, my after lunch blood sugars have been higher than they were before! I do take prednisone and know that the med increases blood sugars--Could it be my morning dose of medicine that's raising my afternoon levels. I've tried changing my diet, etc, but nothing I do makes a difference.

My questions: What could be causing this? What's the next step? The nurse suggested that I may have to take two different kinds of Insulin at the same time. Does that mean two shots? One other question: my MFM doctors are the ones moderating my insulin and controlling my gestational diabetes, but everything I've read from other moms on line indicates that they see an endocrinologist. What is the standard of care for GD? Should I ask to see an endocrinologist or do MFM/OB routinely treat this condition?
