Blood testing is not 100% reliable, and this is a rare, but not unknown situation. Typically, I will repeat the test the third time, and if there is any doubt whatsoever, go ahead a treat with the Rhogam (it won't hurt someone with Rh+ blood). Still, 2 sequential tests that are positive is fairly conclusive.
