Fear of Pain from Post-Birth Surgical Procedures

The major fear that I have is, I had a postpartum hemorrhage about an hour and a half after the delivery, I had already eaten, they tried uterine massage and then started an IV of demoral and then did an emergency D&C. The pain was horrible. I did not scream during the delivery at all, I only grunted a little when my ob used the vacuum extractor. My ob had gone home by the time I hemorrhaged and the chief resident [at a level 3 hosptial in NJ] did the D&C. I couldn't stop screaming and he just yelled that I would die if he didn't do this. About a month postpartum I was pretty much able to block this part out but I am getting quite upset about it now.

When I asked my ob why they didn't use more anesthesia she said I had eaten so general was out and I was already in danger from the hemorrhage, also no time for epidural since I didn't already have one in.

Question---if I get an epidural this time would it provide effective relief for a D&C. I've heard that many times epi's don't completely work so what would happen then.
