Insurance question for the providers?

This may be a somewhat morbid/tacky question, but another post got my curiosity up.

It is my understanding that most insurers pay for OB care under a flat, global fee for a routine, uncomplicated pregnancy. And the doctor gets paid after delivery. So what happens when the baby is lost late 1st trim or early 2nd trim, when you may already have several office visits or even some US scans invested? Does the doctor get the full fee, a prorated version, or nothing at all?

I just wondered if this may be the reason that some docs wait til late 1st trim to schedule the first OB appt?
