Sometimes if epidural medicine is given too close to time to push, it can make it a bit more difficult to push. Most of the time, the urge to push is there when it is time epidural or not. There are lots of options for pain meds during labor, and most at least will do a local to numb up bottom a bit if epsiotomy needed. Talk to you r OB about options for pain control, and also I highly suggest a chilbirth class (Lamaze, Bradley, etc)
