Help with sonogram

Working on baby book and I am trying to firgue out all the mumble jumble
on the sonogram can someone help? The date of the sonogram is dated
10/12/98 but there is something showing 11w2d what is that?I figured
that it stood for 11 weeks and 2 days but of what? From day of
conception? LMP? or is this the age of the fetus? If it's LMP what time
would I figure as time of conception? Baby was born May 1,1999.
