Then when they deny coverage that you think is covered in the contract between you and them that you signed when you bought the policy( or your employer did when they bought the policy), you need to take the complaint to the State Insurance Commissioner or Panel or whatever it is in your state and they can adjudicate the dispute. It is a matter of contract law and what was in the contract that was agreed to. Any contract that was issued when a certain law mandates certain things, must have those things in the contract. RDB
