Some babies are not ready for solids at 4 months. They push their tongue out of their mouth in response to any foreign object put there and gag (this is a reflex to protect little babies from choking). Also, the fact that he has been spitting up since formula was introduced makes me think he might be allergic to the formula. If it's a dairy based forumula, she might try a soy based formula instead. Dairy allergy is very common in little babies.

Also, has she considered pumping when she goes back to work? If the formula is not working it might be an alternative. I've never pumped at work (I'm a stay at home mom) so I don't know how easy/difficult it would be, but I have several friends who have done that.

She should talk to her baby's pediatrician... Probably the only thing wrong with his digestive system is that it is still very immature. All babies develop at different rates, some babies are ready for solids at 4 months and others are not ready until 6 months. If he is not nursing/taking a bottle frequently during the day (more than every 3-4 hours), and not waking at night hungry, he probably doesn't need solids yet. My daughter did not start solids until she was well over 5 months old, and she was a healthy, chubby little thing.
