Naturally it can occur with two ovulations at the same time, and sex with different partners around the same time with fertilization of both eggs. There are also occurences of mistakes made by infertility clinics where there have been inseminations where sperm was mixed, or zygote implantations where the zygotes were mixed. There was a case on "60 Minutes" about a white Dutch woman who gave birth to one white baby and one black baby when she was accidently implanted with one of her own zygotes, and one from a black couple that was there at the same time.
