It is not uncommon to miss the heartbeat by Doppler at this point in pregnancy, but it is expected that an US will be performed, as it was in this case, to provide reassurance both for mother and doctor. Many factors can influence the size of the uterus at 15 weeks, with multiple pregnancies and wrong dates heading the list. While it is possible to miss a multiple pregnancy on US, it is extremely rare, and is not something that you should waste energy worrying about. The US is also quite accurate at dating at this point. The position of the baby at 15 weeks is completely irrelevant. The baby is still a small fish in a biggish pond, and would be expected to be spinning and flipping at will. Since the baby is alone, and the right size for your dates, there is really not much to worry about. The size of the uterus is a very poor predictor of anything prior to late in the 2nd trimester, and I usually do not measure myself until 24 weeks. Good luck with your pregnancy!
