Jody,
You are at 27weeks of gestation ( Parity??) with twins with Hypothyroidism,
chronic UTI,Gestational Diabetes with ketones, chronic hypertension with
rapid pulse and normal BP and abruption. It appears you have autoimmune
disorder that goes in favour of most of the problem you have now.You are at
a very high-high feto-maternal risk for developing PE, IUGR etc., Abruption
is generally common under such circumstances. I am not sure about your
blood loss as well as the extent of retroplacental clot & Complete blood
count ( Haematocrit level), you may be anemic also.You really need bed rest
along with expert perinatal care. BP may or may not rise even then you can
develop severe PE or even Eclampsia with just urine protein rising. I think
you should get full regular biophysical fetal monitoring ( including Doppler
studies) and detailed work up done at the earliest for Liver, renal,
autoimmune, Coagulation functions (HEELP) and also proper Diabetic
assessment both in terms of better control and prognosis. I am sure your
high risk OB must be taking care of you. Follow your OB advice regularly.
