I think that to pick up IR they have to do the Glucose Tolerance Test. They will, in addition to checking blood sugar levels (starting fasting and then at intervals after ingestion of a glucose drink), check your insulin levels. If you have elevated insulin levels and the decrease in blood sugar doesn't mesh with the amount of insulin your body is putting out, then you have IR. In IR, your body puts out more and more and more insulin to counteract the effects of high blood sugar because your body is resistant to the insulin. I hope that made sense. Merely checking fasting levels probably won't tell them what they need to know. My experience has been that my fasting levels weren't remarkable, but when my blood sugar elevated my body was unable to effectively use my insulin, so the tests showed increased insulin levels as well.
