The answer to your question is "there is not a diffinitive test for PCOS". PCOS is diagnosed by physical symptoms as well as bloodwork and family history. Although this syndrome was "discovered" 75 years ago, it has only been in the very recent future that the metabolic connection has truly began to be understood. Because of this, and the fact that the degree of symtoms that present themselves varies so widely, doctors that are not well up to date on current information and treatments can easily fail to diagnose PCOS. There is also many a doctor that does not understand that this is a hormonal and not reproductive sydrome still choosing to "band aid" the symptoms with birth control pills. It would be so nice if PCOS was like a broken leg. Do an xray, find out what wrong, fix it. Even the blood test they use for testing insulin and blood sugar are for diabetics. Hopefully as time passes there will a more exact way to determine if you have PCOS and the "probably" wont have to be used any more.
