If your cortisol levels haven't been tested, I'd say you're definitely not being paranoid. (if they HAVE been and were normal, you're probably ok!) Cushing's is definitely more rare than PCOS, but it is something to keep in mind. I think that's especially true if you tried treating the PCOS and the treatments didn't work - quite possibly then PCOS isn't what's going on. It can be hard to differentiate between the two (women with PCOS can have slightly raised cortisol levels - although not the very high levels of Cushing's - and women with Cushing's can, like all other women, have ovarian cysts. Not to mention the extremely similar symptoms!) but I think in your case, with a non-response to standard PCOS treatment AND a key symptom of Cushing's, you should at the very least be tested. You sound like you agree! Can you push your doc / endo for the 24 hour urine cortisol test?
