Only as far as it puts your wife at higher risk for diabetes and heart disease, and not having regular periods can put her at higher risk for certain cancers. The cancer risk can easily be mediated by inducing a period three times a year or more with birth control / Provera. The other factors improve with improved insulin levels (helped with medication and diet.)

To my knowledge, Syndrome X is basically PCOS without the severe hormonal problems, essentially the same thing. Your wife SHOULD be tested to rule out Cushing's (similar symptoms, adrenal based problem), and probably to have her thyroid checked (commonly a problem with PCOS and similar symptoms.) There may be other things as well but basically, if it looks like PCOS, 99.9% of the time it is PCOS.
