Hi all- I wanted to post here and see if I could get any input. I've had irregular periods since high school which was always attributed to my long distance running-was put on bcp to regulate my cycles. Stopped bcp after 10 years to ttc, no periods on my own since stopping bcp in sept 02. (all have been induced with provera/prometrium). I was originally seeing a ob/gyn but recently switched to an RE and told him I think I have pcos (based on b/w showing slightly elevated testosterone). He agreed...and so it begins!! In any case, I'm now concerned because I had new b/w drawn and my GTT/insulin level results have me totally confused. Everything was normal except my insulin levels at a 1/2 hour and at an hour after drinking the glucola came back on the LOW side (at a 1/2 hour 52.4 uiu/mL with normal being 60-92. At an hour, it was 53.6 uiu/ml with the normal range being 70-102. So one was a little low, the other was pretty gosh darn low. I have elevated androgens-not terrible, but significant. Total testosterone around 61-69 (was done 2x), free testosterone was 13 (normal 1-21 pg/ml), free testosterone and weakly bound was 28 ng/DL (normal listed as 3-29), DHEAS was 385, androstenedione was 470 (pretty high) ng/DL. I'm a relatively lean cyster, no significant hair growth (other than a few here and there unwanted ones but not bad), no signicant acne (other than the occasional pimple or two). If my insulin is normal or LOW based on my test results, where do the elevated androgen levels come from? I thought they were a result of the insulin. I've read that many cysters who don't truly test + for IR probably are to some degree, that the test just isn't sensitive enough. But if mine is normal or LOW, what the heck does that mean? So far I haven't responded well to clomid at 50 or 100 mg for five days. This cycle my RE wants me to do 100 mg cd 3-10 (an extended course). Any input? Should I be concerned about a pituitary tumor? (my prolactin levels are normal). Does it still seem consistent with pcos?? I'm waiting to hear back more from my doc tomorrow about the results.... thanks- ehb (24 y/o ttc #1)
