I'm sorry you are having so much trouble with your weight and your health. It sounds like you could have PCOS from your symptoms.

Your doctor is right about doing an exam. It is helpful to be able to feel the ovaries and see how big they are, if there are any abnormalities that can be felt that way, and also she needs to do a pap smear because it sounds like you haven't had one in quite a while or you have never had one?

I was a virgin until I was 26 and married, so I understand what you mean about dreading exams. But imagine my relief when I found out they can (and do) use a very small speculum, so there is no tearing, and if you relax it only feels like pressure. A good doctor will be finished in just a few minutes (trust me, they don't linger!!). I would categorize it as a minor discomfort.

As to the psychological aspect of it . . . modesty may be a concern of yours. They will cover you up as much as you wish; it's just that they will do it with paper, not cloth. You can ask for an extra cloth "sheet" to put over yourself or around yourself. The key is to try to relax as much as possible. If you need a valium to get through this, then you might want to ask for one.

Your doctor should also take some blood samples to check your hormone levels, and should check your fasting glucose level and your insulin levels. It might be that you are gaining weight due to an overabundance of insulin, which is common with this disease. Some good books to read about a diet that will not make your insulin high include "The Schwarzbein Principal" by Dr. Diana Schwarzbein and "The Carbohydrate Addict's Lifespan Plan" by Drs. Richard and Rachel Heller.
