I found one through a search engine at http://www.e-cureme.com/medical_symptoms.htm , but it seems to be individual symptoms instead of several at once, if that matters! I don't know anything about it so use at your own risk....

There seems to be a really good one at

http://www.medical-library.org/mddx_index.htm with a lot of good questions that your doctor might ask you,

And you might be able to find something through the list of links at

http://www.americanhospitals.com/questions/medical/symptoms.htm

Again I don't know any of these sites personally, but hopefully this will help!
