You will need to check this out with your insurance company yourself. They really individualize the plans, and can also tell you who they cover. You can certaintly call and ask about their benefits. If you have PCOS, they can manage that, and it might not even be considered Fertility. GEtting the PCOS in balance can help with fertility.
