The brown spots could be acanthosis nigricans. It can be a symptom of insulin resistance which may be the cause of PCOS. Here is some additional information and some pictures of acanthosis nigricans: http://dermatology.cdlib.org/DOJvol6num1/original/acanthosis/katz.html
The birth control pill decreases glucose metabolism which may actually be making your PCOS and insulin problems worse.
