You both need to find yourself new doctors pronto. My fasting insulin was lower than yours and I was given glucophage which has made a tremendous difference in my life. Before I started I worked out and dieted very strictly, doing everything right and could not lose a pound. I've now lost 15 pounds and feel much better.

Pick up PCOS the Hidden Epidemic, by Thatcher and/or Women, Weight and Hormones by Vliet
