IR = insulin resistance, which is now thought to be the underlying endocrine disorder that causes PCOS, and can lead to type II diabetes. Testing specifically for IR is at this time very expensive and only done in trials, but certain indications leading to a diagnosis can be revealed with blood glucose tests, fasting tests, and tests for insulin levels. Generally, an insulin level above 8-10 is suspect as being insulin resistant, in varying levels, while a high fasting glucose level can be suspect for diabetes. A good fasting level should be under 100.
