High cortisol levels could indicate Cushings. But high cortisol levels are seen in people in general with insulin resistance. I do rearch on patients with HIV lipodystrophy which involves insulin resistance and some of these patients have high cortisol levels and buffalo humps but they do not have Cushings. As far as PCOS and "normal" blood work, yes it is possible for everything to be in the limits of normal and still have PCOS. Usually it is diagnosed by 1) irregular or absent periods; 2) hyperandrogenism: acne and/or male pattern body hair; and 3) exclusion of other endocrine disorders.
