I'll try again. Maybe someone else can help too.

We need to do two things. Make insulin and get it into the cells. The pills help with using it. If we're not making it (due to type 1 diabetes, or type 2 with worn out pancreas), then we need the shots of insulin.

I think that your question is that if the oral agent worked in the first place, then why would the pancreas wear out at all. Is that a correct interpretation? Well, no med is perfect (don't we all know that). Medical management isn't perfect; not every doctor monitors things as closely as they should, and modifies treatment as often and extensively as the might need to. (We all know that too.) It's not detected as early as it should be. (Again, we understand that.) So, the pancreas is working hard for a period of time before it's controlled. Lastly, people usually aren't as vigillant as they should be when it comes to their self-management, such as eating exactly the right foods, checking blood sugar as often as necessary, proper actvity level, etc. (Again, we know that too.)

So, if everything was done perfectly, the pancreas shouldn't wear out.
