My understanding of it is this: Sometimes your body does not produce enough insulin, and your blood sugar gets too high. That's Type I diabetes. Sometimes your body doesn't use your own insulin effectively (IR.) When that happens, and your blood sugar gets too high, it's Type II diabetes. When that happens, and your pancreas compensates by overproducing insulin until there's enough to keep your blood sugar in a normal range, that's what we deal with in PCOS.

Diabetes is characterized by having high blood sugar. A very high blood sugar level (consistantly over 140mg/dl) would indicate diabetes, and a very low insulin level (<10 mIU/ml I believe) would happen with Type I diabetes.
