One thing to keep in mind with state-based rules is that unless the insurance company is based there, they don't have to conform to them.

At least, that's what my insurance company told me. It's based in GA, but I live in CA, a state that requires insurance companies to allow women to choose an ob-gyn as their primary care physician, but when I tried, my insurance company says that they don't follow state mandates.
