in vitro Maturation - Announcement of a New Program The OHSU-Fertility Clinic is now offering in vitro maturation to women with polycystic ovarian syndrome (PCO). PCO patients are characterized by abnormal endocrine parameters and the proliferation of numerous small follicles without normal follicular growth patterns, and therefore are unable to produce mature eggs. In addition, this group of patients is sensitive to exogenous hormonal stimulation and may be at risk of ovarian hyperstimulation when treated with gonadotropins. In vitro maturation in conjunction with an in vitro fertilization cycle has the potential to generate embryos for transfer to PCO patients without ovarian stimulation and the risk of developing ovarian hyperstimulation syndrome. The procedure begins with an egg retrieval followed by culture for up to 48 hours to allow the eggs to mature. All mature eggs will be inseminated by intracytoplasmic sperm injection (ICSI). Fertilized eggs are cultured until they reach the blastocyst stage on day 5 or 6 when they are transferred back to the uterus. Patients are given estrogen and progesterone to prepare the endometrium for implantation. During this process excess embryos will be frozen so the patient will be able to use them later.
