I think you'll find that Dr. Thatcher's book answers a lot of questions. Metformin, according to Dr. Thatcher, can help almost all PCOS patients, whether or not the have (measurable) high insulin levels. You may also find that your wife's health and fertility are helped by reducing carbs... Read the chapter on hormones and the link to carbs.
