women with polycystic ovary syndrome and hyperinsulinemia, according to
study results published in the June issue of Fertility and Sterility.
Dr. Antoni J. Duleba, from Yale University School of Medicine, New
Haven, Connecticut, and colleagues treated 39 patients with polycystic
ovary syndrome and elevated fasting insulin levels with 12 weeks of oral
metformin, at a dosage of 500 mg, three times a day. Four women became
pregnant during treatment and were not included in the final analysis.
By the end of the treatment period, fasting insulin levels had dropped
by over 30%, while the glucose-to-insulin ratio increased by 33%, the
authors determined. The group also observed an almost 40% decline in
the free-testosterone index. Body mass index and the waist-hip ratio
dropped slightly but significantly.
"Clinical manifestations of hyperandrogenism also improved," the authors
report, although improvement was influenced by baseline DHEA
concentrations. Overall, the acne score dropped by 14%; the hirsutism
score by 2.3%, and the average length of the menstrual cycle by 36%.
Levels of insulin-like growth factor I showed little response to
metformin treatment in women with normal baseline DHEAS concentrations,
but the levels increased by 9.5% in women with high baseline DHEAS
concentrations.
"It appears that patients with adrenal hyperfunction represent a unique
subpopulation of polycystic ovary syndrome," the authors write.
