Hi Lisa- I'm very much like you and suspected strongly that I was having a glucose-related (and perhaps insulin-related) problem. My fasting glucose/insulin always came back fine, and my doctors consistenly pooh-poohed the idea of looking into it further. (Same thing: They'd simply imply that other things could possibly cause it and leave it at that.)

I'm not a medical professional, but from what I understand from others on this board, IR can indeed be missed by the fasting glucose/insulin test. The longer (2 to 5 hour) IGTT (insulin glucose tolerance test) is better. It took me awhile, but I had to find a doctor who was willing to write a script for the longer IGTT . (He's a PCOS expert, and I had to pay for him myself since he's not part of my insurance program. I felt it was worth it.)

Here's something I've learned, though: BE INCREDIBLY EXPLICIT THAT EVERYONE INVOLVED WITH THE TEST UNDERSTANDS that INSULIN should be measured as well! I've taken the test 2 times now and I'm appalled to say that BOTH times a mistake was made at the lab and only the glucose was measured! Now I have to decide it I want to take the darned text yet AGAIN. It's unbelievable.

Interestingly enough, an endo I'm working with now thinks it's a moot point: Some doctors believe that the glucose drink (administered during an IGTT) produces an inaccurate glucose/insulin response. (They prefer to administer a meal--which is set up by a dietitcian--rather than the glucose drink.) You may want to discuss with your doctor what his thoughts are about this before you undergo the test, Lisa.
