I am now 45 and was diagnosed when I was 21 (when the syndrome was
called Stein-Leventhal Disease). In spite of extreme anemia and
numerous other symptoms between onset in puberty and diagnosis, no
diagnostic efforts were made until I was experiencing extreme and
frequent pain. After diagnosis and surgery, I was told the pain had
been from multiple bursting follicles. So, yes, the "cysts" can be very
painful, even though much of the literature says they're not. I still
get occasional pain, though nothing like what it was back then. It is
not accompanied by breakthrough bleeding -- though that is now; I do not
recall whether it was all those years ago (I don't think so).
