Sallie, could you tell me the name of the Medscape article about adolescents and PCOS and IR? Let me update you on Rachel, my 16 year old. (Background: she had always been thin. After stopping the birth control pill, a year ago she went from a size 3 to a 12 in a few months. Body hair growth escalated, along with hair loss on her head, insomnia, etc. Finally, after 4 doctors in San Diego missed it, we went to Tucson, saw Dr. Elizabeth Vliet, who instant;y diagnosed her with PCOS from her blood work. Started Ovcon35, spironolactone in August, metformin in October (after the GTT/Insulin test). In January she went to 1,500mg of met.) Now the good part: Her body is returning to normal. She has lost 15 pounds since January without following a strict diet. She watches her carbs and always puts protein with any food. She has always exercised. All of a sudden, the weight if just coming off. Hair growth is way less. Texture is softer. Complexion is flawless. I am convinced that this all happened with metformin. It didn't help right away. She went up very slowly but it took the therapeutic dose to do it. I had heard Dr. Nick Cataldo speak at the Oregon conference in October and he told me that less that 1,500mg probably wouldn't help her. Rachel and I went to San Jose to hear him again at the San Jose Chapter meeting in December (thanks, Heather!). It helped me push to get her to 1,500mg, which he says is the therapeutic dose. I realize that metformin might not help everyone- I've heard 50% of the people- but it has helped her!

(By the way, she is on the Ovcon35 birth control pill because she has very low estradiol and this is the best way of replacing it for her. It's a very good pill. We have had some glitches here, with breakthrough bleeding, pain, etc that we are fine tuning.)
