I wrote INSMED about the difference in d chiro inositol (INS1) and just plain inositol (myo-inositol). The reply was that d chiro inositol is a natural substance that INSMED has exclusive rights to test. I asked if plain inositol would work and they said that they did not know because they were not testing that. In articles I have read d chiro inositol or INS1 (what they call it) calls d chiro inositol a natural substance because it is a something the body can make. Inositol is changed in the body into many different forms of inositol. The way I understand it is either blocked from making d chiro inositol out of inositol or it is just changed to form that is not normal - by insulin - and causes the hormone changes in PCOS and diabetes. D chiro inositol is supposed to be a big plus for diabetics also. When you take inositol on an empty stomach inositol is not changed by insulin or digestive enyzmes and can therefore make the types of inositol that are needed to complete hormonal pathways. I can't explain it all here because I would need to write a book but you can learn more about inositol by reading IP6 - here is the website http://www.ip-6.net/secondpage.html here is INSMED's site http://www.insmed.com/ You can also put inostiol in a search engine or go to PUBMED and read research studies on inositol. I went to the bookstore and read on inositol in all the books. Hope this helps some
