First, don't fret! If she does have PCOS, it's rare that you would not improve with treatment---it's all downhill from here--it just gets better, her health will improve, her mood will improve, etc.. all good things.

Those are some major symptoms of PCOS..of course, a doctor could be the only one to diagnose it but I would say its very likely--I am 19 years old adn I started showing signs of PCOS at 8 or so and really heavily around age 12 (weight gain), and didn't get diagnosed until 16.

Other things can cause those symptoms, though, so do keep that in mind. I would recommend that you call your general practitioner and ask as if you are not a current patient if the doctor is familiar with PCOS and insulin resistance. If no, you may consider finding a doctor in your area who is--just call around. Essentially, she'll end up going to an endocronologist and a gynecologist. The general practitioner can do the blood work--they need to check alot of things, the main ones coming to mind are free testosterone, insulin levels, blood sugar, chloresterol/lipid panel, thyroid just to be safe, and they may request a cortison test (you would collect urine for 24 hours and bring it back to the lab, but I think many of us on the board did not under go this), she would also have an ultrasound done and if they request it to be vaginal you can tell them you'd prefer they do non vaginal and live with the not as clear results if she's a virgin or is uncomfortable with that. I also had to have an adrenal cat scan.

So, you're looking at alot of testing but once that's through I can't imagine anything but improvements. It's wonderful you noticed these symptoms and researched it--that's a huge step in the right direction for managing PCOS.
