PCOS with no medication

How soon should a person with PCOS start taking Metformin or Clomid or other medication? Say for example, you plan to get married and have kids in 3 years, when is the best time to start medication for PCOS? Meanwhile, if you are not trying to conceive, and have regular periods, is it okay to just leave your PCOS alone and not take any medication? Does BBT charting work for people with PCOS? How do I find a PCOS expert in the Philippines? Had 3 obgyns already, none of them had expertise in PCOS and could not refer PCOS expert. Thank you.
