What test did they do? If she had fasting insulin only, she should have either 1) fasting glucose AND fasting insulin to see whether the ratio between the two is less than 4.5 (which indicates insulin resistance), OR 2) she should have a 2 or 3-hour fasting glucose with insulin test. At least I think those are the right testing protocols.

Getting on an insulin sensitizing medication such as metformin or Glucophage could help her lose weight in conjunction with exercise and moderating her carbohydrate intake. There's a new book out now called something like the PCOS Diet Book that she might find helpful. The insulin sensitizing drugs also help with the other symptoms you mention, because high insulin is now thought to cause the excess male hormones that result in acne, high BP, facial hair, etc.
