A low carb diet can range from very strict to not very strict. There are books like atkins (very strict) up to sugar busters, and the zone (not so strict). Also a diet based on low glycaemic index foods may help.

Probably the easiest way to start is by cutting out any/most refined carbs (ie. white bread, white rice, pasta, noodles, starchy vegetables, highly processed foods, soft drink, fruit juice etc). Your diet should then consist of vegetables, complex and wholegrain/high fibre carbs, healthy fats and protein. One priniciple is to make sure you eat some fat, protein and carb at every meal. The fat and protein will help slow down the carb absorption and result in more stable insulin levels. If you are following a low calorie diet a good multivitamin is probalby recommended.

You are already exercising. That is good. Keep it up.

Perhaps some topic meds can help with the acne. I also have the same problem. Drives me nuts - and then I pick at them (BIG NO NO) and make it worse. I have been using diane-35 (a bcp) with moderate success - but am taking a break for now.

A drug such as metformin (insulin sensitiser) may be helpful - at least for a trial.

If you want to try bcp again, you may try a different type. Side effects can be different with different pills.

If you eat so low calorie and still cannot lose wieght, make sure that your doctor has screened you for thyroid problem.

I beleive that there is a tendency for PCOS to get worse over time, though not in all cases. However research on this is still in its early stages. THe effect of treatm,ent/managmenet in preventing this getting worse have still to be defined.

Your chances of getting pregnant are most likely good. PCOS is one of the most treatable of infertility conditions (and not all women are infertile) - sometimes they say reduced fertility, rather than infertility.

A really good book is called PCOS - the hidden epidemic by Dr. Sam Thatcher.
